Create a New Form in Service-Up

  1. Login to https://up-family.com.au/
  2. Click on Up-Family logo and click on Service Up.
  3. In Service Up dashboard, click on Forms present on the left hand panel.
  4. Click on Create Form button and select the type of form you want to create (Example: Jerry Site Checklist Daily).(Note: Different types of form will have different fields to fill)
  5. Click on custom Form, Jerry Site Checklist Daily.
  6. Complete the form with necessary information and add Signature.
  7. Once completed, click on Save button.
